If you wish to enroll your dependent children, you must complete the enrollment form and return it along with your initial $90 dependent coverage payment to the fund office at P.O. Box 4148, Portland, OR 97208, no later than December 1, 2024.
COVERAGE FOR YOUR DEPENDENT CHILDREN IS ONLY AVAILABLE DURING MONTHS IN WHICH YOU ARE ELIGIBLE FOR COVERAGE FROM THE FUND
Si desea inscribir a su hijo (s), debe completar el formulario de inscripción y entregar lo junto con su $90 de pago cobertura de dependientes inicial a la Oficina del Fondo al P.O. Box 4148, Portland, OR 97208, a más tardar el 1 de diciembre 2024.
